Thyroid cancer stem cells and thyroid cancer treatment: theory and applications

Abstract:

BACKGROUND: Thyroid cancer stem cells are one of the reasons for tumor resistance that promotes tumor development. The research of thyroid cancer stem cells has provides a new clinic means for the diagnosis and treatment of thyroid cancer.

OBJECTIVE: To overview the discovery, identification, and correlation of thyroid cancer stem cells with thyroid cancer.

METHODS: A computer-based online search of PubMed database and Wan Fang database between 1995-01/2014-01 was performed to search related articles with the key words of “thyroid cancer, cancer stem cell, stem cell, cancer suppressor gene” in English and Chinese, respectively. Literatures related to thyroid cancer stem cells were selected; in the same field, the articles published lately in authoritative journals were preferred.

RESULTS AND CONCLUSION: A total of 561 literatures were primarily selected, and 57 documents were involved for result analysis according to inclusion criteria. Cancer stem cells have become a focus in the study of carcinogenesis. Stem cells also exist in the thyroid gland and its tumor. At present, there are several ways to isolate and identify thyroid cancer stem cells. The tumor thyroid stem cells are closely related to the occurrence, transfer and treatment of tumors. But whether we can cure thyroid cancer through restraining or eliminating thyroid cancer stem cells is still unknown that needs further studies.

2.1 甲状腺肿瘤干细胞理论的提出及相关研究甲状腺肿瘤的发生可能与饮食中的碘缺乏或过量、遗传、放射或环境、地理因素刺激等有关。很久以来大家都认为肿瘤是体细胞在上述因素刺激的作用下突变而来。但肿瘤中只有一少部分的肿瘤细胞能够无限制地自我更新，多潜能分化并且产生多种肿瘤细胞来形成肿瘤[1]，这就是肿瘤干细胞。肿瘤干细胞被定义为肿瘤中具有自我更新能力并能产生异质性肿瘤细胞的细胞，与干细胞相比有许多共同的特性[2-3]，并且可能在癌症发展中发挥作用[4-5]。依据肿瘤干细胞的特性可以通过不同的方法分离肿瘤干细胞，如诱导EMT过程、化学药物刺激及侧群分选等方法[6-8]。目前已经从白血病、成胶质细胞瘤、乳腺癌、胃癌和结肠癌中发现并分离出肿瘤干细胞[9-13]。2007年，Mitsutake等[14]首次从甲状腺肿瘤组织中分离出具有自我更新及多向分化的潜能的侧群细胞。随后，Todaro等[15]的研究进一步证明了在甲状腺癌中存在肿瘤干细胞。这些发现改变了关于肿瘤发生机制和肿瘤治疗策略上的一些观点，给临床肿瘤治疗带来新的方向。 2.2 甲状腺肿瘤干细胞标记物鉴别和认识这种具有致瘤作用的肿瘤干细胞，对寻找和发现更多地针对控制甲状腺肿瘤干细胞治疗甲状腺肿瘤的方法非常重要。在甲状腺肿瘤干细胞的研究中发现，表达相关标记分子如CD133、CD44、CD24、ABCG2的这类细胞，同时也表达干细胞多能性的基因如Sox2、Oct4和NANOG，而低表达或不表达甲状腺分化标志的基因如HNF4α，GATA4，PAX8和TG，TPO，TSHR和NIS等[16]。其中CD133是一种越来越受欢迎的甲状腺肿瘤干细胞标记分子[17]，甲状腺肿瘤中的CD133阳性细胞可以表现出干细胞样的特性，如增殖扩散，自我更新，克隆形成及对化疗药物(多柔比星，顺铂和依托泊苷)的抗性[18]。此外，CD133和TITF-1还可以分别特异性地标记甲状腺未分化癌和甲状腺髓样癌[19]。一些特殊的标记，如：经免疫组织化学染色发现，巢蛋白可能是甲状腺未分化癌干细胞最特殊的标志物[20]；从甲状腺乳头状癌组织中分离出的甲状腺样的球状细胞团，高水平地表达胰岛素受体亚型、胰岛素样生长因子1受体、胰岛素样生长因子1和胰岛素样生长因子2，但在分化中的细胞中却明显减少[21]。然而，也有不能完全被肯定的甲状腺肿瘤干细胞标记物如NANOG[22]，有待相关研究的进一步确认。通过对这些甲状腺肿瘤干细胞特有标记物的鉴定，早期地识别并分离肿瘤干细胞，就有望对肿瘤做出早期诊断，以标记物为靶向进行定向的根除治疗，可以为复发性和难治性甲状腺癌的治疗带来希望。 2.3 甲状腺肿瘤干细胞与甲状腺肿瘤发生发展的关系甲状腺肿瘤干细胞是甲状腺肿瘤组织中的一群具有无限增殖、自我更新和克隆生长等干细胞行为特征的特殊细胞。研究发现甲状腺肿瘤干细胞在甲状腺肿瘤的启动、复发、转移以及肿瘤的抗放化疗中均有作用。 2.3.1 甲状腺肿瘤干细胞有促进甲状腺肿瘤发生及复发的潜能传统的观点认为肿瘤是由成体细胞突变而发生。但是细胞恶性转化是一个多基因、多因素、多阶段的复杂病理过程，而成体细胞很少分化并且寿命短，很难积累到发生肿瘤所需的突变。在同种异体间输入外周血细胞可能会引起肿瘤的发生[23]。因此推测，肿瘤的发生更多的是由干细胞的突变导致，肿瘤细胞可能来自组织固有的未成熟的祖细胞或干细胞[24]，也可以通过上皮细胞间质样转化引起[25]。甲状腺肿瘤被认为是由甲状腺肿瘤干细胞产生的异常组织。 Ahn等[22]从甲状腺乳头状癌中提取培养的甲状腺样的细胞团，能够引起免疫缺陷小鼠肿瘤的发生。这些细胞团在有侵袭和复发倾向的甲状腺乳头状癌中高表达CD44和CD24。一些表达干细胞标记(CD133、POU5F1和NANOG)的细胞也能够在免疫缺陷的小鼠中启动肿瘤或者强化肿瘤的启动[26-28]。体内131I治疗甲状腺未分化癌细胞株ARO，导致细胞CD133、Oct4、NANOG、Lin28、Glut1基因的表达增加，在分离出的甲状腺乳头状癌培养的细胞中CD133阳性细胞表现出较高的抗辐射性和对Oct4、Nanog、Lin28、Glut1基因的表达增强，却对甲状腺特有的基因如IS、Tg、TPO、TSHR、TTF1和Pax8表达降低，表明CD133阳性细胞在放射治疗的微环境下具有更大的生存潜能，很可能是甲状腺癌复发的原因所在，未来抗放射性的甲状腺癌的治疗方法更可能关注并选择对CD133阳性细胞的根除治疗[29]。甲状腺肿瘤干细胞可以诱发甲状腺肿瘤，但是具体机制尚不十分明确。ARID1A是一个在癌症中频繁缺失的基因位点，目前被认为是一种重要的抑癌基因。研究发现ARID1A在未分化的甲状腺癌的缺失比例为14%[30]。ARID1A低表达与癌症复发、浸润、淋巴结及远端转移相关[31-32]。ARID1A的低表达可以促进肿瘤细胞增殖、侵袭、转移及抑制细胞凋亡。ARID1A作为SWI/SNF复合物的一个亚基，能够影响SWI/SNF复合物对下游基因位点的靶向性。SWI/SNF复合物的下游靶基因，包括干细胞自我更新基因Sox2、Utf1和Oct4等[33]。SWI/SNF失活导致基因突变不断累积。这有可能是肿瘤干细胞致瘤性的原因所在。 2.3.2 甲状腺肿瘤干细胞可能是甲状腺肿瘤发生转移的原因之一肿瘤转移是一个复杂的病理过程，包括肿瘤细胞脱离原发灶，侵袭血管或淋巴系统，最终到达其他组织器官形成转移病灶，而肿瘤干细胞是这整个过程的重要因素。上皮细胞间质样转化(epithelial- mesenchymal transition，EMT)是上皮源肿瘤向间质细胞变性的一种现象，在对乳腺癌，卵巢癌，胰腺癌和前列腺癌的研究中发现，上皮细胞间质样转化使癌细胞具有干细胞样特性[34-37]，是癌细胞对肿瘤发生、转移、侵袭性、耐药性作用的一个主要机制[38-40]。二者之间相互作用的机制目前已有一定的进展。上皮细胞间质样转化的发生与钙黏蛋白的基因表达缺失有关，在这个过程中，细胞失去黏附性和极性并获得入侵表型。Montemayor-Garcia等[41]通过定量PCR检测甲状腺癌中的Smad7 mRNA，发现其在甲状腺未分化癌中表达显著高于其他组，推测Smad7的表达增加可能与甲状腺肿瘤进展有关。并发现甲状腺未分化癌细胞中钙黏蛋白的表达减少，ZEB1的表达增加，证明甲状腺未分化癌细胞显示上皮细胞间质样转化。Vasko等[42]关于乳头状甲状腺癌的研究表明，该肿瘤的特点是上皮细胞间质样转化对波形蛋白的超表达，该蛋白与侵袭性和淋巴结转移相关。然而甲状腺乳头状癌、滤泡癌以及滤泡腺瘤及正常甲状腺均匀表达钙黏蛋白阳性[43]，这支持上皮细胞间质样转化在未分化甲状腺癌发展中的作用。上皮细胞间质样转化程序可能是甲状腺肿瘤干细胞发生的重要驱动力。研究表明：HIF-α能够诱导人甲状腺癌细胞完成上皮细胞间质样转化，从而促进肿瘤干细胞的发生[44]，为甲状腺癌的远处转移和治疗耐受提供了条件。Mani等[45]实验证实，人类乳腺上皮细胞模型显示细胞间充质特征的获得与干细胞标记物的表达有关。CD44是肿瘤干细胞和上皮细胞间质样转化的标记，CD44 RNA干扰可以阻止甲状腺肿瘤细胞的增殖，这表明CD44-CREB信号通路可以维持肿瘤细胞的增殖[46]。进一步研究上皮细胞间质样转化与肿瘤干细胞的关系，有可能开发出遏制肿瘤干细胞发生的新型抗肿瘤药物，为临床治疗肿瘤带来希望。 2.3.3 甲状腺肿瘤干细胞可能是肿瘤耐药的分子事件甲状腺癌尤其是未分化甲状腺癌对化学治疗不敏感。除了上述的上皮细胞间质样转化为肿瘤的耐药性提供可能外，另有一些研究发现肿瘤耐药可能与肿瘤细胞的某些通道作用密切。异种移植甲状腺髓样癌与化疗药物5-氟尿嘧啶在体内作用后，这些细胞对CD133的表达增加。将甲状腺髓样癌细胞株TT同5-氟尿嘧啶体外培养后，发现这些细胞表现出对5-氟尿嘧啶显著地药物抵抗性[27]。在Carina等[47]的研究中，所有甲状腺未分化癌病例显示OCT-4、KLF4、SOX2转录因子和C-KIT和THY-1干细胞表面抗原的表达上调，SOX2关闸通过ABCG2输送下调在克服肿瘤干细胞对化疗药物耐药性方面有作用。ABCG2是一种与多药耐药性相关的蛋白，可以泵出许多癌症药物的ATP-结合载体。这些发现有助于了解抗肿瘤药物治疗中存在抵抗性或耐药性的一些可能机制，提出治疗策略的新挑战。 2.4 甲状腺肿瘤干细胞在肿瘤治疗中的研究临床上治疗甲状腺肿瘤的方法(手术、放疗、化疗)进十几年内没有基本没有变化，都能迅速杀灭已分化的癌细胞，但仍有少数静止的潜在肿瘤细胞被存留下来。对甲状腺肿瘤干细胞的研究意义不仅在于发现其在肿瘤发生发展中的潜在能力，更重要的是如何将这些细胞应用于靶向、安全、有效、个体化抗肿瘤治疗和甲状腺的再生。由于肿瘤干细胞能通过多种机制逃避药物诱导的细胞死亡信号，如上调 ATP结合转运蛋白，激活DNA的修复能力和过表达抗凋亡分子等，因此这种针对肿瘤干细胞的理想药物是不容易实现的。 2.4.1 阻止肿瘤干细胞增殖分化，诱导肿瘤干细胞去干细胞特性样分化在甲状腺未分化癌的研究中发现，分离出的ATC-CD133阳性细胞相比未分离的ATC-CD133细胞，其致瘤性、抗辐射性、对胚胎干细胞和耐药基因的表达都增加。用抑制剂葫芦素Ⅰ治疗ATC-CD133阳性细胞，发现其肿瘤干细胞样的能力减弱，相反，该细胞对甲状腺特有的基因表达上调，且摄取碘的能力增强，相对而言增加了ATC-CD133阳性细胞对放化疗的敏感性。研究者通过异种移植实验表明抑制剂葫芦素Ⅰ加上放射性化学疗法可以显著抑制肿瘤的发生，提高被移植ATC-CD133阳性细胞小鼠的生存率。研究还发现ATC-CD133阳性细胞中STAT3通路在调停肿瘤干细胞属性中发挥着关键作用[48]。针对抑制剂葫芦素Ⅰ在甲状腺未分化癌STAT3通路的作用，可能会为未来的治疗提供新的方法。甲状腺肿瘤的发生发展与其肿瘤干细胞中多种信号通路的过表达或缺失有关，抑制或修复某些信号通路可能给甲状腺肿瘤的发生发展带来一定的影响。甲状腺肿瘤干细胞表达CD44，许多化学阻断剂如RET/PTC，BRAF或者是金属蛋白酶可以将CD44表达减弱。而CD44-CREB是维持肿瘤增殖的信号通路，CD44 RNA的干扰调停反应也可以降低或阻止甲状腺癌细胞的增殖[46]。甲状腺肿瘤干细胞的致瘤性和高侵袭性与 Met/Akt 信号通路有关。在对甲状腺肿瘤干细胞的基因转录程序的研究中发现：将沉默了Met和Akt的未分化癌细胞转入小鼠的甲状腺组织中后，发现沉默的Met和Akt能将甲状腺肿瘤干细胞的潜在侵袭能力基本消除[15]。此外，甲状腺肿瘤细胞对ABI3BP的重复表达减低导致细胞的生长、生存、迁移和侵袭能力降低，还可以触发p21细胞衰老途径。肿瘤的发生发展可能与肿瘤干细胞ABI3BP的表达缺失有关，激活ABI3BP或其通路可能会为癌症定向治疗提供基础[49]。研究发现二甲双胍通过抑制胰岛素或胰岛素样生长因子信号通道抑制细胞克隆生长，减少甲状腺样球状的干细胞团的形成，加强化疗药物如阿霉素在甲状腺未分化癌细胞中的抗诱变效应的影响[50]。该研究表明二甲双胍可以明显减少胰岛素升高对甲状腺的刺激。而通过药物阻止甲状腺肿瘤干细胞增殖分化并不能彻底防止甲状腺肿瘤的复发，杀伤已分化细胞的同时破坏潜在突变的肿瘤细胞是最理想的治疗方法。上述的这些疗法对有活跃转移灶的侵袭性癌症患者，是否可以增加其作用持续时间或患者的生存质量，有待进一步研究证明。要改进对癌细胞转移过程的研究，以发现治疗甲状腺恶性肿瘤的新疗法。甲状腺肿瘤干细胞的研究已经得到一定的进展，但是由于甲状腺肿瘤干细胞的临床应用仍面临着很多问题，目前还没有一种肿瘤干细胞治疗方案应用于临床。甲状腺肿瘤干细胞研究成果主要从动物和体外实验中获得，目前为止还不能将研究成果直接应用于人体；由于肿瘤干细胞与正常干细胞有很多相似之处，在针对肿瘤干细胞治疗中必须确保对肿瘤干细胞的特异性杀伤；由于肿瘤干细胞仅是肿瘤组织中的一小部分细胞群体，被杀伤后并不会短时间内出现明显的肿瘤缩小等较明显反应，因此，需要确切的评价指标来评价肿瘤干细胞治疗的疗效。 2.4.2 甲状腺的再生一些甲状腺恶性肿瘤给患者带来不可复性损伤。而许多器官进行部分切除后，剩余部分的细胞会发生肥大或增生,特别是肝脏进行部分切除后，会显示出显著的再生能力。而甲状腺进行甲状腺部分切除术后，剩余组织没有表现出显著的大小变化[51]。类比肝部分切除术，可用于学习和了解甲状腺部分切除术后甲状腺再生的机制。再生甲状腺也将成为提高患者生存质量最理想的治疗手段。上述的研究中可以发现，通过一些手段可以使甲状腺肿瘤干细胞去干细胞样特性，增强其对甲状腺样细胞特性的表达。但是甲状腺肿瘤干细胞诱导后其分化方向不确定，因此，甲状腺的再生需要依托更为安全、可靠的干细胞系。目前只有胚胎干细胞在诱导分化为甲状腺细胞方面有一定成果。Reigh和Arufe的研究发现体外培养的胚胎干细胞可以分化成甲状腺滤泡样的细胞集落[52-53]。E14鼠的胚胎干细胞在体外经过促甲状腺激素、胰岛素及碘化钠的诱导，可以分化为甲状腺细胞[54]。但是介于伦理道德等问题，胚胎干细胞的研究受到一定的限制，而其他干细胞如来源广泛、取材容易的脂肪间充质干细胞[55]，可塑性强、无致瘤性、免疫原性低的脐血干细胞，已经在治疗某些疾病中展示出一定疗效的多能诱导干细胞[56-57]，以及新发现的羊膜干细胞等，是否具有分化为甲状腺细胞的能力，仍需要进一步的研究。"
